Most weight-loss assessments are 40 questions designed to qualify you in or out of a prescription. Ours is shorter, more thoughtful, and built around understanding your actual life: your goals, your medical history, your relationship with food and your body, what you've tried before, and what's working now.

You won't be charged at this stage. If you're not a clinically appropriate candidate, a physician will explain why and may suggest alternatives. If you are, your program begins within 24 hours.

A minority of applicants aren't clinically appropriate for compounded GLP-1 therapy, most often due to medication interactions, family history, or pre-existing conditions that require in-person care. Your assessment isn't auto-approved by an algorithm. A board-certified physician at our affiliated medical group, Openloop Healthcare Partners, PC, reviews your full profile, your medications, your history, and your goals — and decides whether compounded GLP-1 therapy is clinically appropriate for you specifically. If it is, they prescribe a starting protocol calibrated to your response.

You'll receive a secure message with the physician's decision and their reasoning. If they have questions before deciding, they'll message you directly. If you're approved, your protocol begins within days.

You can request one through your care team at any time, and we'll route your case to a different physician in our network. There's no charge for second opinions. You also keep all your records and can share them with your primary care doctor whenever you'd like — we encourage it. Elara is part of your care team, not a replacement for it.

Read by a real physician.

Most telehealth GLP-1 programs prescribe from a form. We prescribe based on your actual metabolic biology. A physician orders a comprehensive panel that goes to any Quest Diagnostics or Labcorp location near you. The visit takes about fifteen minutes. You don't pay extra. You don't fight with insurance.

When your results come back, a physician reviews them in context with your assessment and explains what they mean in plain language — fasting glucose, HbA1c, lipid profile, liver and kidney markers, thyroid status. Labs repeat quarterly so the trend is visible, not just the snapshot.

A physician will explain exactly what's flagged, whether it changes your protocol, and whether it warrants additional follow-up. Sometimes unusual labs mean we adjust your starting dose. Sometimes they mean we order additional tests before proceeding. Sometimes they mean we recommend you see a specialist before continuing. In every case, you'll get a clear explanation, not a confusing report to interpret alone.

The point of quarterly labs isn't to watch a number.

Your compounded medication is prepared by a state-licensed 503A compounding pharmacy operating under state pharmacy board oversight. It ships in unmarked, temperature-controlled packaging. The first shipment includes everything you need: vial or tablets, supplies, an instructional card, and a one-page protocol specific to your prescription.

If you chose oral semaglutide, you'll receive tablets and a daily protocol. If you chose tirzepatide, both injection and oral formats are priced the same. Refills ship automatically every 30 days. If you need to pause, switch formats, or adjust, you message your care team and it's done.

Oral semaglutide and oral tirzepatide are both available as daily tablets, so you have needle-free options if injections aren't a fit for you. Talk to your physician during intake about which format makes sense given your lifestyle, preferences, and clinical profile — the right choice depends on your specific situation, not a one-size-fits-all recommendation.

Two weeks after your first dose, a certified health coach reaches out. From then on, you meet every three weeks by secure video or phone. Your coach helps you build the habits that make the medication actually work: nutrition that complements appetite changes, movement you'll actually do, sleep, stress, and the small daily decisions that compound over time.

Coaches read the physician's notes before every session, so you never have to repeat yourself. Between sessions, you can message the care team anytime.

You can pause coaching sessions for up to 60 days at a time without affecting your medication subscription. Just message your care team. We don't lock you into a session schedule that doesn't fit your life, and the option to pause is always there when you need it. When you're ready to resume, your coach picks up where you left off — no re-explaining, no starting over.
