Somewhere between week 8 and week 16 on a GLP-1 medication, a meaningful percentage of patients notice more hair in the shower drain, on their pillow, or in their brush. The shedding often peaks just as the weight loss is becoming visible to other people, which makes the timing feel cruel. The patient is doing everything right, the medication is working, and now their hair is falling out.
This guide explains what’s actually happening physiologically, why GLP-1 therapy can trigger it, the nutritional deficiencies that make it worse or longer, and the evidence-based steps to address it. The short version: this is almost always treatable, often preventable, and never as catastrophic as it feels in month three.
What’s Actually Happening: Telogen Effluvium
The clinical name for the shedding most GLP-1 patients experience is telogen effluvium (TE). It’s one of the most common forms of hair loss in adults and is well-documented in any situation involving rapid weight change, nutritional shifts, or significant physiological stress — pregnancy, post-surgery, severe illness, crash diets, and now GLP-1 medications.
Here’s the physiology: at any given time, roughly 85–90% of your scalp hair is in the active growth phase (anagen), and 10–15% is in the resting phase (telogen). GLP-1 medications don’t directly cause hair loss. The medication doesn’t act on hair follicles. What’s happening is downstream of the weight loss itself, and there are typically three overlapping mechanisms:
1. Rapid weight loss as a physiologic stressor
Losing weight quickly — even healthy weight that the patient wants to lose — is metabolically disruptive. The body reads rapid caloric deficit as a stressor and shifts resources away from non-essential processes. Hair growth is one of the first systems the body deprioritizes during stress, because hair is not essential for survival.
This is why hair shedding is documented in essentially every form of significant weight loss: bariatric surgery, severe caloric restriction, dramatic dietary changes, and now GLP-1 therapy. The faster the weight loss, the more pronounced the shedding tends to be.
2. Protein deficiency
Hair is made primarily of keratin, a structural protein. Producing new hair requires adequate dietary protein. When appetite drops on GLP-1 therapy and patients eat substantially less than they used to, protein intake often crashes — usually without the patient realizing it.
The threshold matters here. Most adults need at least 0.8–1.0 grams of protein per kilogram of body weight per day for baseline tissue maintenance. During weight loss, the requirement climbs to 1.2–1.6 g/kg to preserve lean tissue, including hair. A patient who used to eat 90g of protein daily and now eats 45g is meaningfully underfeeding the systems that produce hair.
3. Micronutrient deficiencies
Several nutritional deficiencies are independently associated with hair shedding, and all of them are more common during caloric restriction:
- Iron / ferritin. Iron is required for hair follicle function. Ferritin (the body’s iron storage protein) below 30 ng/mL is associated with hair shedding even when hemoglobin is still normal. Many adults — especially menstruating women and patients on low-meat diets — have low ferritin without knowing it.
- Vitamin D. Vitamin D receptors are present on hair follicles, and deficiency is linked to multiple forms of hair loss. Most adults have lower-than-optimal vitamin D, and intake doesn’t increase on a restricted diet.
- Vitamin B12. B12 deficiency contributes to hair shedding and is more common in people who eat less meat or have GI absorption issues. GLP-1 patients may absorb less B12 due to slowed gastric emptying.
- Zinc. Zinc is required for protein synthesis including keratin production. Deficiency is associated with hair shedding and is more common in restricted diets.
- Biotin. Often supplemented heavily for hair health, biotin deficiency is actually rare. The supplement industry has overstated its role. Adequate but not megadose biotin is what matters.

The Clinical Timeline
Telogen effluvium has a remarkably consistent timeline that helps patients calibrate expectations:
- Weeks 0–8: No noticeable hair changes. The stressor (rapid weight loss) is happening, but the affected hairs are still in the resting phase.

What to Do About It
1. Get the right labs done
The single most useful intervention is testing the deficiencies that drive shedding. The right panel includes:
- Complete blood count (CBC) with attention to hemoglobin and hematocrit
- Iron studies — ferritin, serum iron, TIBC, transferrin saturation. Ferritin is the most important number; under 30 ng/mL warrants supplementation, and many dermatologists target 70+ for active hair regrowth.
- Vitamin D (25-hydroxy). Target above 30 ng/mL minimum, ideally 40–60.
- Vitamin B12. Target above 500 pg/mL for hair health, even though “normal” range extends lower.

- Total protein and albumin. A snapshot of protein status, though not a perfect measure of dietary protein adequacy.
- Zinc, if shedding is severe or persistent.

2. Hit your protein target consistently
Target 1.2–1.6 grams of protein per kilogram of body weight per day. For a 180-pound patient, that’s 100–130 grams daily, distributed across meals (at least 25–30 grams per meal to trigger protein synthesis). Practical strategies:
- Lead with protein at every meal. Eat it first, before you fill up on other foods.
- A whey or plant protein shake daily is one of the simplest ways to add 25–30g of complete protein without needing a large appetite.
- Greek yogurt, cottage cheese, eggs, and lean meats deliver more protein per volume than most other foods — useful when appetite is suppressed.
- Track for 30 days. Most patients underestimate their protein intake. An app like MyFitnessPal or Cronometer will show whether you’re actually hitting the target.
3. Address specific deficiencies
If labs come back showing deficiencies, address them under physician guidance:
- Low ferritin: Iron bisglycinate is typically better tolerated than ferrous sulfate. Pair with vitamin C for absorption. Avoid taking with calcium or coffee. Retest in 8–12 weeks.
- Low vitamin D: Vitamin D3 with vitamin K2 cofactor. Dose depends on the deficit and is best individualized with your physician.
- Low B12: Methylcobalamin (the active form) is preferable to cyanocobalamin. Sublingual or injectable for severe deficiency.
- Low zinc: Zinc picolinate at 15–30 mg daily for a defined period (not indefinitely; long-term excess zinc can cause copper deficiency).
Don’t supplement based on guessing. Test first, treat to the result, then retest.
4. Give it time
This is the hardest part. Even when you do everything right — protein, labs, deficiency correction — the shedding wave that’s already underway has to finish. New hair takes months to grow back. Patients who started addressing the underlying causes in month three may not see meaningful regrowth until month seven or eight. This is biological, not a sign that what you’re doing isn’t working.
5. When to see a physician

- Scalp is red, painful, scaly, or itchy — this suggests a separate dermatologic issue

What Not to Do
- Don’t buy expensive “hair growth” shampoos. Most are marketing. The cause of TE is internal, not topical. Shampoo can’t reach the follicle in any meaningful way.
- Don’t take megadose biotin “just in case.” Biotin deficiency is rare. Excess biotin can interfere with several common lab tests (including thyroid and troponin), causing false readings. If you take a biotin supplement, stop it 72 hours before any blood work.
- Don’t add five different supplements at once without testing. If you’re shedding and have low vitamin D, treating the vitamin D is targeted and useful. Throwing biotin, collagen, marine extracts, and “hair vitamins” at the problem masks what’s actually working and can introduce side effects.
- Don’t stop your GLP-1 medication preemptively. TE is time-limited. Stopping the medication won’t reverse shedding that’s already in progress, and the rapid weight regain that often follows is its own stressor. Talk to your physician before changing your protocol.
- Don’t compare your scalp to before-photos obsessively. Mid-shedding is the worst possible time to evaluate. Wait until the wave has passed and regrowth has had several months to fill in before drawing conclusions.
